Cocaine is a drug of current health concern because of its proliferation among recreational drug abusers.

 

Freebase and crack increase the potential for major cocaine toxicity. Cocaine use is declining across the nation according to the National Institute of Drug Abuse.

 

Increasingly, laboratory results are disputed or there are medical/legal overtones. Therefore, physicians are finding an increased need to confirm positive results before informing or confronting the patients.

 

Chain of custody is a record of the disposition of a specimen to document each individual who collected, handled, and performed the analysis. When a specimen is submitted in this manner, analysis will be performed in such a way that it will withstand regular court scrutiny.

Interpretation

Reports will specifically indicate the presence or absence of cocaine and benzoylecgonine.

 

The presence of cocaine, or its major metabolite, benzoylecgonine, indicates use within the past 4 days.

 

Cocaine has a 6-hour half-life, so it will be present in urine for 1 day after last use.

 

Benzoylecgonine has a half-life of 12 hours, so it will be detected in urine up to 72 hours after last use.

 

There is no correlation between concentration and pharmacologic or toxic effects.

Method Description

Drug classes are initially screened by an immunoassay technique.

The cocaine assay is based on the kinetic interaction of microparticles in a solution as measured by changes in light transmission. In the absence of sample drug, soluble drug conjugates bind to antibody-bound microparticles, causing the formation of particle aggregates. As the aggregation reaction proceeds in the absence of sample drug, the absorbance increases. When a urine sample contains the drug in question, this drug competes with the drug derivative conjugate for microparticle-bound antibody. Antibody bound to sample drug is no longer available to promote particle aggregation, and subsequent particle lattice formation is inhibited. The presence of sample drug diminishes the increasing absorbance in proportion to the concentration of drug in the sample. Sample drug content is determined relative to the value obtained for a known cutoff concentration of drug.(Package inserts: Roche Cocaine. Roche Diagnostics; 03/2019)
